PROGRAM BENEFITS FOR CLUB AZURE

Use of the Club AzureMembership Card constitutes acceptance of the conditions listed herein:
1. Membership Card: The Club AzureMembership Card is the property of the issuing agent and must be returned upon request. Membership privileges will only be honored upon presentation of a valid Membership Card and only through the date embossed on the Membership Card.Membership Card has no cash value.

2. Dining Benefits: Upon presentation of your valid membership card each Club Azure member will be provided a complimentary meal when accompanied by one (1) or more dining guests, provided at least one meal is purchased. The complimentary portion of your bill is determined by dividing the total food bill (excluding alcoholic beverages, taxes and gratuities) by the number of people in your dining party, and subtracting that amount from the original bill. Limit one (1) card per table.

Dining benefits are non-transferable. Your card will be honored at the The Shores Resort & Spa at Azure for breakfast, lunch and dinner. The Shores Resort & Spa reserves the right to change hours of operation without prior notice. Not valid with any other discounts or promotions. For reservations, call 386-767-7356.

3. Dining Restrictions: Dining privileges will not be honored at the The Shores Resort & Spa on Valentine’s Weekend, Easter Sunday, Mother’s Day, Fourth of July, Thankgsiving Day, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day or The Shores Resort & Spa Wine Festival. Other blackout dates may apply. Please contact the The Shores Resort & Spa to confirm the date each holiday is observed. Not valid with any other discounts or promotions.

4. Guest Room Benefits:
A) Each Club Azure member is entitled to a Preferred Membership Rate, at least 10% off best available daily rate. Subject to availability. A valid membership number must be presented in order to redeem benefit, however, member need not be present at his/her guest check-in to qualify for the above rates. Transferable.
B) Each Club Azure member receives one (1) certificate valid for one (1) complimentary night stay. This non-transferable benefit is valid based on advance reservations and restrictions on allocated rooms. Subject to availability. Reservation must be made 14 days of arrival. In order to redeem this certificate, member must contact The Shores Resort & Spa directly at 386- 767-7350. If cancelling, member must do so within 72 hours of the reservation.
C) Each Club Azure member receives two (2) "3 Nights for 2" certificates each valid for a complimentary third consecutive night's stay providing the previous two (2) nights are paid for. Not valid in conjunction with Complimentary Night Stay Certificate or Preferred Membership Rate. Subject to space availability. Reservations must be made in advance. Certificate must be presented at the time of check in. Transferable.
D) Each Club Azure member is entitled to a complimentary upgrade to the next available room category when using the Complimentary Night Stay certificate. Subject to availability. Non-transferable.
E) Each Club Azure member is entitled to a complimentary upgrade to the next available room category when using the Complimentary Night Stay certificate. Subject to availability. Non-transferable.
F) Each Club Azure member receives one (1) certificate entitling the bearer to have the Resort Fee waived. Not valid in conjunction with Complimentary Night Stay Certificate. Subject to space availability. Reservations must be made in advance. Certificate must be presented at the time of check in. Transferable.

5. Guest Room Restrictions: Guest room benefits are valid for standard guest rooms only, based on advance reservations, subject to space availability and restrictions on allocated rooms. Not valid for bookings through travel agents, group or block bookings, or in conjunction with corporate rates.Membership number must be provided upon time of advance reservation for member and/or guest. Certificate and Membership Card must be presented in order to redeem benefits.Where benefits are “transferable”, member and Membership Card need not be present at time of guest check-in to qualify for the above benefits.

6. Birthday/Anniversary Benefit: Each Club Azure member receives one (1) certificate valid for one (1) complimentary specialty dessert while enjoying dinner at Azure during the week of the member’s birthday or wedding anniversary. Not valid for room service. Non-transferable.

7. Catering and Banquets Benefit: Each Club Azure member is entitled to 15% off Green Fees and Cart Rentals at LPGA International Golf Club, 1000 Champions Drive, Daytona Beach, FL 32124. Non-transferable. In order to redeem this certificate, member must contact the course directly at 386-274-5742.

8.Wine Makers Benefit: Each Club Azure member receives one (1) certificate valid for a 25% discount off any one (1) of the Wine Makers Dinner series. Valid for member and up to three (3) guests. Subject to availability. Not valid with any other discounts or promotions. Non-transferable.

9. Complimentary Drinks Benefit: Each Club Azure member receives one (1) certificate valid for two (2) complimentary drinks at the Tiki Bar at any time the bar is open for business. Member must be 21 years of age to order drinks. Not valid with any other discounts or promotions. Non-transferable.

10. Complimentary Champagne Benefit: Each Club Azure member receives one (1) certificate valid for one (1) complimentary glass of house champagne when dining in Azure. Member must be 21 years of age to order champagne. May be used with member discount. Transferable.

11. Corporate Meeting Benefit: Each Club Azure member receives one (1) certificate valid for 25% discount off a Corporate Lunch Meeting (excluding taxes and gratuities). Includes a plated lunch and a choice of one (1) preferred visual aid: flip chart, slide projector or overhead projector. New bookings only. Minimum booking of ten (10) people and a maximum booking of twenty five (25). Reservations must be made in advance. Not valid with any other discounts or promotions. Non-transferable.

12. Azure Bar Benefit: Each Club Azure member is entitled to 15% off for all food (excluding taxes and gratuities). Not valid during happy hour and black out days. Non-transferable.

13. Breakfast Club Meeting Benefit: Each Club Azure member receives one (1) certificate valid for 25% discount off a Breakfast Club Meeting (excluding taxes and gratuities). Choice of any breakfast on the catering menu and one (1) preferred visual aid: flip chart, slide projector or overhead projector. New bookings only. Non-transferable.

14. Complimentary Business Breakfast Benefit: Each Club Azure member receives one (1) certificate valid for a complimentary business breakfast for two (2) people (excluding beverages, taxes and gratuities). Transferable.

15. Spa Benefit:
A) Each Club Azure member is entitled to 20% off spa treatments in the Spa Center. Advance reservations required at 386- 722-7232. Excludes service charge and retail products. Valid Monday to Friday. Non-transferable.
B) Each Club Azure member will receive one (1) certificate valid for 10% of Spa products. Taxes excluded. Not valid for Spa
services. Non-transferable.

16. Barista Benefit: Each Club Azure member receives two (2) certificates each valid for one (1) complimentary specialty espresso or coffee beverage. Not valid for alcoholic beverages or cash redemption. Only one (1) certificate can be used at a time. Transferable.

17. Member for a Day Benefit: Each Club Azure member receives one (1) certificate valid for Club Azure member dining benefits for the entire day. Transferable.

18. Golf Benefit: Each Club Azure member is entitled to 15% off Green Fees and Cart Rentals at LPGA International Golf Club, 1000 Champions Drive, Daytona Beach, FL 32124.Non-transferable. In order to redeem this certificate,member must contact the course directly at 386-274-5742.

19. Referral Card Incentive: Upon submitting five (5) names of friends or colleagues to be solicited for potential memberships, you will receive one (1) certificate valid for one (1) complimentary bottle of house wine, pursuant to Florida liquor laws, or dessert for up to two (2) people to be enjoyed while dining at Azure during dinner. Member must be 21 years of age to order wine. Bottle may not be removed from restaurant. Non-transferable.

20. Certificates: The THE SHORES RESORT & SPA must be notified as to the use of certificates at time of advance reservation. Certificates must be presented upon arrival in order to redeem benefits.Valid only through the date embossed on Membership Card.

21. Other Promotional Discounts: Club Azure membership privileges may not be used in conjunction with any other discount
programs or promotions, including corporate rates.

22. Lost/Stolen Cards and Certificates: All lost or stolen certificates will not be replaced. Lost or stolen Membership Cards must be reported. A processing fee of twenty dollars ($20.00) will be charged for replacement of Membership Cards. If your card is lost or stolen, a replacement card will be issued with a new membership number and the same expiration date as the previously issued card.

23. Cancellations: Cancellations will be honored up to ten (10) days from the date of purchase, but such termination will not be effective until all cards and membership materials have been returned unused to the Club Azure Membership Processing Center, 1045 Howe Street, Suite 925,Vancouver, BC, Canada, V6Z 2A9. No refund will be payable until such action has been taken by the member. Please note that the five dollar ($5.00) shipping and handling fee will not be refunded in the case of a cancellation.

24. Limitation of Liability: In no event will the marketing agent of Club Azure be liable for any indirect, special, or consequential damages without limiting the generality thereof, including any loss, cost or damage to or suffered by Club Azure members or others, arising from defects, negligence, delays, failure of performance or non-performance. Including all claims, demands, obligations, liabilities, indebtedness, breaches of contract, breaches of duty or of any relationship, liability for acts, omissions, misfeasance, malfeasance, damages of every type, nature, or kind suffered by the Club Azure member or to others, resulting from the action, or inaction of the Hotel or its agents or employees.
